The New England Journal of Medicine article “Health Consequences of Immigration Enforcement in U.S. Communities” by Melissa Aguello Belli, M.D., argues that immigration enforcement policies have measurable effects on community health and access to medical care. Enforcement actions — and even the perceived threat of them — create fear and uncertainty that discourage many immigrant families from seeking medical treatment. This dynamic affects not only undocumented individuals but also mixed-status families, including U.S. citizens, leading to delayed care, canceled appointments, and reduced use of preventive services.

The article highlights several health consequences associated with this climate of fear. Patients often postpone routine visits, diagnostic testing, and chronic-disease monitoring, which can worsen medical conditions and lead to preventable complications. Children in affected households experience increased anxiety, sleep disturbances, behavioral regression, and school difficulties. Researchers have also linked immigration enforcement exposure with higher rates of depression, trauma-related symptoms, and chronic stress among both children and adults.

The author concludes that immigration enforcement should be viewed not only as a legal or political matter but also as a public-health issue. Disruptions in care, declines in preventive health programs, and strained relationships between patients and health systems can weaken already stressed medical infrastructure. The article calls on health systems and policymakers to recognize immigration enforcement as a social determinant of health and to develop strategies that maintain access to care, protect patient trust, and mitigate avoidable health harms in affected communities.
